Living with a chronic illness means navigating a fragmented healthcare system and managing multiple specialists, evolving symptoms, and medical records scattered across disconnected portals. Chronius Health set out to give patients one place to understand their care and advocate for themselves at every stage.

I designed the patient portal so that lab results, family history, clinical notes and appointment records all live in one place. The Patient Journey Map helps patients understand where they are in a typical care trajectory, from first symptom through diagnosis and into long-term management. Patients can also find doctors who specialize in their specific condition, schedule and manage appointments, and share relevant records ahead of visits.
Track symptoms without feeling overwhelmed
I designed the companion app, a symptom-tracking experience, around a short, focused list, so patients can log what they're feeling, its intensity and relevant context in real time. Those entries feed directly into the Health Summary Report and can be reviewed in a journal format to help identify trends or triggers over time.
A health summary report built for an actual patient appointment
I designed the Health Summary Report to capture a snapshot of a patient's health journey tailored by specialty. Instead of starting from scratch at every appointment, patients walk in with a report that surfaces key milestones, recent symptoms and goals, giving doctors what they need in limited time.
